Area II. Topic H. Root Cause Analysis
Introduction
"Root causes often relate to the management system. They may be due to management's policies and procedures, supervision and its effectiveness, or training. Root causes are those which would effect permanent results when corrected. They are those weaknesses which not only affect the single accident being investigated, but also might affect many other future accidents and operational problems." — Safety Management : A Human Approach, Dan Petersen, ASSE Pub.
